Radiological Materials Licensing

No radioactive materials may be ordered or possessed prior to obtaining a Radioactive Materials License. Qualifications and required experience for licensees obtaining or working with radioactive materials, and other guidance for preparing an application, can be found in the following US Nuclear Regulatory Commission guidance documents:
Proof of Worker's Compensation and Disability Insurance, including the name of the insurance carrier, the policy number, and the expiration date must be submitted when you apply for a permit and when you renew. Or, if exempt, a copy of Form CE-200 stamped by the Worker's Compensation Board with the New York State-assigned Exemption Certificate Number. The certificate must list the New York City Department of Health and Mental Hygiene as the Certificate Holder.

Print a copy of the Application for Radioactive Materials License. Note that sections 7-14 of the application describe documentation that must be submitted along with the application form.

Complete the application and mail it, along with all required documentation to the Office of Radiological Health at the address below.
