This Certificate of Fitness can only be obtained by passing both a written and verbal exam. All applicants should review the Notice of Exam and Study Materials.

A completed CERTIFICATE OF FITNESS APPLICATION FORM (PDF)
A letter of employment.
SAMPLE EMPLOYER RECOMMENDATION LETTER (PDF)
SAMPLE SELF-EMPLOYMENT LETTER (PDF)
If required, a copy of your diploma or academic transcript.
Two forms of identification, at least one must be a government-issued photo identification, such as a driver's license or a passport.

To take the verbal exam, applicants must have passed the written exam and have received a temporary Z-83 letter.
