Before you submit your application
Blank Access Control Card must be obtained from your Market employer before submitting your application for processing. This Blank Access card must be submitted together with the application.
Proof of Residence, which may include a Copy of a Utility bill (telephone/gas/electric), current lease agreement, or current mortgage payment. If the utility bill is not under applicant's name, a notarized statement form the person whose name appears on the bill is required, indicating the applicant resided at the address.
Proof of Identification , which may include a City, State of Federal issued identification card, or valid certificate of birth (marriage or court documents if name on birth certificate has changed), Military discharge papers , passport or naturalization certificate
Photo to be taken at the time of application submission
Proof of Social Security Number.

Steps to apply in person:
1.
Complete the application for Class A Photo Identification, linked above.
2.
Submit the original and one copy of completed application and all required documentation and required fees in person to the Business Integrity Commission Command Center located at the New Fulton Fish Market at 800 Food Center Drive, Bronx, N.Y. 10474.
3.
Once an application has been reviewed and accepted, the applicant will remain in a pending status until completion of a background investigation.
4.
Applicants will be scheduled by the license unit to come in and have their fingerprints taken for processing. At the time of the appointment, the applicant will be required to submit a money order for $75.00 made payable to: NYS Division of Criminal Justice.
