Need birth or death certificates to help fill your family tree?

We can help if you are related to the deceased and:

• You need a death certificate.  Ordering online is easiest.
• You need a birth certificate and
  • Your relative died in New York Cty.  Bring or mail us a photocopy of his/her death certificate in addition to filling the other requirements for getting a birth certificate.
  • Your relative was born in New York City but died somewhere else.  Bring or mail us his/her original death certificate addition to filling the other requirements for getting a birth certificate.
  • You indicate "family tree" in item #10 when you complete the birth certificate application.
