You will be required to provide specific information on your application form. Please gather this information before you begin the application process. To identify the information you will need to have available, click the name of the license(s) for which you will apply, below. For most applications, you will need the following information:
Contact information
Social Security Number (SSN) or Individual Tax Identification Number (ITIN) (for sole proprietors and individual partners)
History of criminal and / or civil charges of the Business Owners, Partners, Shareholders, and Officers
Child support obligations (for sole proprietors and individual partners)
For your application to be processed, you must pay the licensing and service fees using a major credit card.
Incomplete applications cannot be saved.
You will need to have a printer accessible to print your confirmation page after submitting your application.
Your session will time out after 30 minutes of inactivity.
