SCHEDULE YOUR OWN APPOINTMENT
Welcome to the Licensing and Standards Appointment Scheduling Site
You can schedule an appointment to submit an application for the following transactions:
Purchase a Wheelchair Accessible or Non-Wheelchair Accessible Street Hail Livery permit
Changes to a Street Hail Livery permit such as:
Affiliate to a Base Transfer to a new SHL Base Update ownership, address or contact information
For-Hire Vehicles, Paratransit or Commuter Vans Transfer Application
Update your name and/or photo on your TLC Driver License
Submit a Base or Business application
When you visit the self-scheduling site, you will be able to select the appointment date and time of your choice.

Information you will need to provide when scheduling an appointment:
Telephone/Cell phone number
Valid email address
TLC License Number (for current licensees)
An appointment confirmation with the date and time of your appointment will be sent to the email address you have provided. Keep this email for your records.
Information you will need to provide on the day of your appointment:
Your appointment confirmation email. You will not be processed without it.
A completed and signed application.

Cancellation / Rescheduling of appointments
Appointments that need to be cancelled or rescheduled must be done no less than 48 hours prior to the appointment. To cancel or reschedule your appointment you must use the link found in the original appointment confirmation. An email confirmation with the cancellation and/or rescheduled date and time will be sent to you.
BASE PLATE NOTIFICATION ONLY for Street Hail Livery (SHL):
If you have submitted your vehicle application to the TLC, you are affiliated to a Street Hail Livery (SHL) Permit, and you are ready to Hack Up your vehicle, you do not need to schedule a full NYS DMV inspection appointment. Go to your base to notify the TLC that you have received your license plates. You will receive a Hack Up letter in the mail. Bring your vehicle with the Hack Up letter to our Inspection Facility at Woodside and the inspection will be completed there.
Base Affirmation: The Base owner will need to submit an Online Base Affirmation before the Vehicle Owner can make the Payment online and submit any documents. To submit an Online Base Affirmation, please go here.
