Child Support Form 
Under state law, the TLC must verify whether individuals who become TLC licensees, including by buying a taxicab medallion, are current with their child support obligations.  Download the form here:


Lien Lawsuit and Judgment Searches 
Under the new rules, all sellers and transferors must provide to the TLC a New York State UCC lien search, together with a lawsuit and judgment search for all counties in which the seller/transferor has been domiciled for the shorter of either five years prior to the transfer, or the period of actual ownership.  The results of these searches must be submitted to the TLC together with an affidavit regarding these searches, signed by both the buyer and seller.  Download the affidavit form here:


Non-reliance Affidavit 
Buyers and other transferees of medallions must affirm that they do not rely upon any representations by the TLC regarding the value of medallions, and they must acknowledge that medallion ownership is subject to all rules and regulations of the TLC, as may change from time to time.  Download the affidavit here:


Notice of Proposed Claim Amount 
If outstanding, unresolved tort claims against the seller have been identified (which typically, but not always, will arise from accidents involving the taxicab), the medallion may not be transferred until each claim has been evaluated, and an escrow established for any amount of the claim that exceeds the medallion’s insurance coverage.  The seller/transferor and the claimant may agree on an escrow amount at any time, but the seller/transferor can begin the formal process to have the claim evaluated by giving notice to the claimant.  Download the notice to the claimant here:
