Forms:

EAS Short Form – This form can only be used for Unlisted actions. Instructions.
EAS Full Form – This form is for all Type I and Unlisted actions. Instructions

Lead Agency Letter – This model document provides guidelines for what a Lead Agency Letter must contain.

Negative Declaration – This template contains language required in a Negative Declaration.
