The following forms are for the use of parties in cases pending before OATH. These forms may be downloaded or printed, completed, and then submitted to OATH in person or by mail. Please note that the intake form is generally for use of City agencies only, since OATH cannot process complaints from the general public and cases are generally referred from other agencies. If you have any questions about OATH's jurisdiction, please write or call OATH's calendar unit.
