 The Applicant Processing Division has provided various forms, which you must prepare prior to reporting for your formal interview with your investigator. Follow the instructions and prepare these forms neatly, carefully, and completely. If you have any questions on how to complete a form, call your investigator or the Candidate Service Desk at (718) 972-2503/2545. DO NOT MAIL THESE FORMS; ALL FORMS ARE TO BE RETURNED TO YOUR INVESTIGATOR. For further instructions please refer to the Instructions for the Screening Packet form (APD/PS-13) that was included in the investigative packet that you received at the initial screening. These forms can be downloaded and completed directly on your computer. You need Adobe Acrobat Reader to view and edit these forms.
