Residents age 65 or older, persons of any age seeking Medicaid related to a disability or blindness, and persons who are in receipt of Medicare and are not a parent or caretaker relative of minor children have the option of applying for public health insurance programs such as Medicaid or the Medicare Savings Program by mail. Consumers are no longer required to complete face-to-face interviews. Application assistance, however, remains available at all Medical Assistance Program Community Offices on a voluntary basis.
If you are unable to visit an office yourself, a relative, friend or any person familiar with your situation may come for you. (You must supply them with a written authorization to represent you.) If you have no representative, you may request application assistance at home.

Find your nearest Medicaid Office below. NOTE: Offices are open from: 9:00 AM to 5:00 PM Monday through Friday. Certified Application Counselors (CACs) are available at select offices to assist individuals (generally those under the age of 65) who must now apply for public health insurance through the New York State of Health (Marketplace).
