Long Term Care Program

The Long-Term Home Health Care Program (LTHHCP) is also known as the “nursing home without walls” program. This program offers a coordinated plan of medical, nursing and rehabilitative care provided at home to persons who are medically eligible for placement in a nursing home. These individuals must have care costs which are less than the nursing home cost in the local county. In addition to all regular Medicaid services, the following may be available:

  • Case management by RNs
  • Home delivered or congregate meals
  • Housing improvements and moving assistance
  • Respiratory therapy
  • Medical social services, nutrition and dietary services
  • Respite care, social day care and social transportation

Services in the home are provided through SDOH-certified LTHHCP provider agencies. HCSP determines eligibility, assesses the client’s service needs and authorizes payment for the approved plan of care, which might include some special waiver services that are not ordinarily covered by Medicaid. In addition, Home Care Services Program participates in the periodic reassessment of the services provided. The providers are responsible for obtaining physician orders and administering the assessment tools.

Individuals can access this program through a hospital discharge planner, HCSP, or a Long Term Home Health Care Provider.
