If you are unable to apply at one of the two DSNAP locations, you can go to one of seven Restoration Centers to designate a representative from a Community Based Organizations based at the centers to be your Authorized Representative to assist you with the application process. This person can take your completed application to a DSNAP location for processing and an eligibility determination.
The Authorized Representative must:
Have your written permission to come to one of two DSNAP locations to file the DSNAP application that you completed on your behalf
Bring proof of identity
Bring proof of your identity
Bring relevant documentation that you have to support your application
Be knowledgeable about your circumstances and be able to answer questions about your income and resources, the number of people in your home, and storm-related expenses
