NYC HRA D-SNAP

D-SNAP Authorized Representative Info

If you are unable to apply at one of the two DSNAP locations, you can go to one of seven Restoration Centers to designate a representative from a Community Based Organizations based at the centers to be your Authorized Representative to assist you with the application process.  This person can take your completed application to a DSNAP location for processing and an eligibility determination.

The Authorized Representative must:

  • Have your written permission to come to one of two DSNAP locations to file the DSNAP application that you completed on your behalf

  • Bring proof of identity

  • Bring proof of your identity

  • Bring relevant documentation that you have to support your application

  • Be knowledgeable about your circumstances and be able to answer questions about your income and resources, the number of people in your home, and storm-related expenses
