To find out if you are eligible for public health insurance such as Medicaid, Family Health Plus or the Medicare Savings Program, you have the option of applying by mail. Consumers are no longer required to complete face-to-face interviews. Application assistance and interviews, however, remain available at all Medical Assistance Program Community Offices and at the offices of all Facilitated Enrollers, on a voluntary basis.

If you are unable to visit an office yourself, a relative, friend or any person familiar with your situation may come for you. (You must supply them with a written authorization to represent you.) If you have no representative, you may request application assistance at home. If you choose to apply by mail, a postage paid return envelope will be enclosed in the application kit for your use. If you misplace the envelope, here is the address to which you should address your return:
