Managed Long-Term Care Program

The Managed Long-Term Care program (MLTC) assists people who are Medicaid-eligible, or eligible for both Medicaid and Medicare, and who are chronically ill or disabled, need health and long-term care services, and who are eligible for admission to nursing homes, but choose to stay in  their homes and communities as long as possible. Plan offerings vary by client’s age and geography but currently most plans accept applicants 18 years of age or older.  Exceptions include:

Senior Health Partners – age 55
Our two PACE plans (ArchCare and PACE) – age 55

Services may include, but are not limited to:

  • Nursing
  • Physical therapy
  • Occupational therapy
  • Speech pathology
  • Medical equipment and supplies
  • Podiatry
  • Dentistry
  • Optometry
  • Respiration therapy
  • Transportatioy
  • Social Day Care

A person can access services by contacting the MLTC plan directly. Referrals to the plan may be received from a physician, another health care provider, or HCSP. The Home Care Services Program then determines Medicaid eligibility and reviews the appropriateness of the enrollment.
