Residential owners may download and print the Carbon Monoxide Detector Certificate of Installation (in PDF) or submit the Carbon Monoxide Detector Certificate of Installation online. Click here to go to the application HPDOnline. When you enter the HPDOnline application, type in your building address. On the next screen, look for the left menu bar and click on the option "Carbon Monoxide Certificate." Then proceed to complete the online Carbon Monoxide Detector Certificate of Installation.
CERTIFICATION OF CORRECTION OF VIOLATION(S) (in PDF) This form is for use when all violations on a Notice of Violation cannot be certified at one time. This form must be completed properly, including identifying the original NOV ID, signing the form and having the form notarized, for it to be accepted by HPD. The form must be returned to the same borough office indicated on the original Notice of Violation and will only be accepted for timely certifications (certifications submitted by the certification date identified on the original Notice of Violation.)
Department of Health and Mental Hygiene (DOHMH) Work Notification Form (in PDF): Building owners must use this form to give 10 days advance notice to the DOHMH prior to undertaking work that disturbs more than 100 square feet or replaces two or more windows in an apartment with a child under 6, where the building was either (a) built before 1960 or (b) built between 1960 and 1978 and the owner knows there is lead paint.
Contestation of Lead-Based Paint Violation (Form L06): Under Local Law 1, HPD may issue presumed lead-based paint violations. These violations have an order # of 616 associated with the violation. If a presumed lead-based paint violation is issued, you will receive with the Notice of Violation a form called an L-06. Form L06 allows you to contest the violation using XRF testing or, in certain cases, paint chip sampling (read the form carefully for details).
Property Registration: Online Property Registration Form: HPD is pleased to introduce our new online property registration form for residential owners. By completing your registration form online, you will instantly be alerted of any errors or information missed. You are still required to print the form and mail the signed copy with payment to HPD. Please note that the information you input will not be saved. To access the form, please click on the link above, input your property's street address and borough location, and click "search". On the screen that follows, click the Property Registration Assistance link located in the left-hand column.
Affidavit in Lieu of Registration: The attached form is used when applying for a deed with NYC's Department of Finance if the property classification does not require that a property registration be filed with HPD. This form is submitted directly to the Department of Finance.
