Community Health Advocates

New York State Department of Health From this website you can review the 2012 New York State Managed Care Plan Performance Report (in PDF) designed to help consumers make informed decisions about about choosing a health plan.  The report contains valuable information, such as a plan’s delivery of primary and preventive health visits, access to health care, and medical management of select chronic diseases. You can also review  eQARR 2012 an interactive report card designed to help consumers evaluate health plans operating in New York State.
