In a show of innovation and leadership, HHC this month received nearly $13 million in State Department of Health grants for projects to help New Yorkers with schizophrenia, diabetes and other chronic conditions. The $10 million health information technology grant will advance a medical home model for patients with schizophrenia and support the ability to share medical information across all types of providers involved in the care of a single patient to improve the patient's coordination of care. Another $2.8 million will support construction and renovation of spaces at Kings County Hospital and Woodhull Hospital Center to allow for more efficient coordination of care for patients with diabetes and other chronic diseases.
