a Continuum of Care
At Coler Goldwater, we recognize that each patient/resident
is an individual with a unique set of needs. Each patient/resident is assessed,
upon admission, for their potential for discharge to the most appropriate setting.
Whether they require placement in our nursing facility or a plan to return to
the community after rehabilitation, the social work and discharge planning staff
is available to assist them. The discharge planning process provides:
- Patient and family education and planning
with assistance programs.
- The acquisition of special equipment and supplies
to facilitate patients mobility and independence.
- Coordination with home healthcare services
and community-based providers.
