Child Health Plus (CHP)
Child Health Plus is a comprehensive health insurance plan available to most uninsured children under the age of 19. Income requirements make it possible for working families to get free or low-cost health insurance for their children. All children are eligible regardless of their immigration status - even undocumented children are eligible.

Family Health Plus (FHP)
Family Health Plus is a public health insurance program for adults between the ages of 19 and 64 who do not have health insurance but have incomes too high to qualify for Medicaid. Family Health Plus is available to single adults, couples without children, and parents with limited income. You must be a resident of New York State. Eligibility based on immigration status will need to be determined by one of our financial counselors.

Medicaid
Medicaid is a program which provides medical assistance for certain individuals and families with low incomes and resources. You may eligible if you have high medical bills; receive Supplementary Security Income (SSI); or meet certain income, resource, age, or disability requirements.

Medicare
Medicare is a federal health insurance program for people age 65 or older or people under age 65 with certain disabilities including End-State Renal Disease. Some people qualify for both Medicare and Medicaid. Medicare now offers prescription benefits as well.

Prenatal Care Assistance Program (PCAP)
PCAP provides expanded Medicaid coverage for pregnant women and for children under the age of 19 regardless of their citizenship status, as follows:

  • Full Medicaid services for pregnant women with incomes at or below 100% of the Federal Poverty Level (FPL), and
  • Comprehensive perinatal services for women with incomes between 100% and 200% of the Federal Poverty Level (FPL).

Woman, Infants and Children (WIC)
WIC is not an insurance plan, but provides free, healthy foods for pregnant or breast-feeding women and children under the age of 5. You receive checks every month to buy the food you and your children need, ideas on how to prepare healthy and tasty meals for your family, and help in getting other healthcare services you need.

HIV Special Needs Plans (HIV SNP)
An HIV Special Needs Plan (HIV SNP) is a special health plan for people on Medicaid living with HIV/AIDS and their children up to age 19, whether or not the children have HIV or AIDS. The doctors, nurses, and other care providers who participate in HIV SNPs understand the special needs facing people living with HIV/AIDS.

Artist Access Program
The Artist Access Program offers services designed specifically to meet the needs of artists. The program provides financial counseling by specially trained staff familiar with artists' employment cycles to determine eligibility for various insurance options - including government programs, or a sliding-scale fee-for-service arrangement. In addition, the program allows artists to exchange performances or other types of artistic activity for health care credits that can be used to pay for doctor's visits or prescriptions. If you are not currently covered by an insurance plan, you may still schedule an appointment with one of our physicians. At the time of your appointment, a financial counselor will meet with you and determine your eligibility for various insurance and payment options. A variety of options are available depending on your age, immigration status, and income. Everyone, regardless of ability to pay, can obtain financial assistance to see a provider at one of our facilities.

What Documents to Bring
The following documents are needed to determine eligibility for insurance programs or for Medicaid recertification:

  • Proof of age, such as a birth or baptismal certificate;
  • Proof of citizenship or alien status;
  • Four (4) recent paycheck stubs, if you are working;
  • Proof of your income from other sources, such as Social Security, Supplemental Security;
  • Income (S.S.I.), Veteran's Benefits (V.A.), and retirement;
  • Bank books and insurance policies you may have;
  • Proof of your residence, such as a rent receipt, landlord statement, or current postmarked envelope;
  • Insurance benefit card or policy if you have any other health insurance;
  • Medicare benefit card; and
  • Support statement, if you are currently unemployed and not receiving any of the above benefits

Prescription Assistance - There are several programs to help Bellevue patients pay for prescription medication.

Prescription Assistance Programs
Patients without prescription coverage are asked to pay a $10 administrative fee for each prescription. Patients will not be asked to pay more than $40 at any single visit, no matter how many prescriptions they receive. If a patient is unable to afford the fee, a Financial Counselor is available to discuss payment options.

Medicare Part D
Medicare prescription drug coverage is insurance provided by private companies that have been approved by Medicare. As of January 1, 2006, new Medicare prescription drug coverage is available to everyone with Medicare. This drug coverage may help lower prescription drug costs and help protect against higher costs in the future.

EPIC
The Elderly Pharmaceutical Insurance Coverage (EPIC) Program is a New York State-sponsored prescription plan for senior citizens who need help paying for their prescriptions. Eligibility is based on income. Seniors who receive full Medicaid benefits are not eligible for EPIC benefits. Seniors with other prescription coverage can join EPIC to pay for drug costs not covered by the other plan.
