Please Note: Filming in city parks, interiors within city buildings, bridges or tunnels will require additional permissions from the controlling entities. Please call the MOFTB to obtain specific contact information.

The numbers on this guide correspond to the item numbers on the MOFTB permit application form. Careful use of this guide will save you time.

Item #1: Company: Production Company name must match the first listed name on the certificate of insurance. Production contact: This is the person acting as representative when we need to contact your company, usually the production office coordinator. Students: Your company name is your school, and must match the name on your school letter. You are the contact - include your phone number.

Item #2: Address and phone number: This should be local information. If your production office is based outside of New York, please list your local hotel or residence. Also include your official address and phone. Students: List your home address, not the school address.

Item #3: Complete the application form and list your locations and times on the "Schedule A" form.

Item #4: Animals, Firearms, Special Effects, or Unusual Scenes: Provide a brief description in this field, and complete detail on "Schedule A" form. Special permits may be required for certain animals. Firearms must come from a licensed dealer. Special effects and Pyrotechnics require Fire Department assistance. Check with the NYC Fire Department Explosives Unit: (718) 999-1595. Any use of firearms, police uniforms, police vehicles, driving shots with tow or camera rigs, or when hazardous conditions or holding of traffic are expected, police assistance must be requested on the Schedule A form. It is your responsibility to confirm your plans with the Movie/TV Police Unit (212) 489-6710, x219 by 10AM of the business day prior to any shoot day requiring police assistance. Please advise the Movie/TV Unit of weekend filming by Thursday at 10AM.Student films requiring police assistance must work Mon.-Fri. only.

Item #5: Child Performers: If "yes" is checked, specify the child's age and activity on the "Schedule A" form.

Item #6: Equipment format: Choose camera type (ie, 16mm, 35mm, Video, Digital) Any special equipment, such as cranes, rain towers, dolly track, etc, that will impede use of sidewalks or roadways must be listed on a Schedule A. # in Cast & Crew : List number of people involved in the shoot, to include production personnel, cast members. Any extras should be listed on the Schedule A as part of the scene description. For parking privileges that accompany this permit, all production vehicles must be listed on the Drop/Add form. Your top sheet, appropriate Schedule A's and production parking tags must be displayed in vehicles that have been registered on the permit and the Vehicle Drop/Add form.

NOTE: No 24-hour parking.

Item #7: Place an X in the appropriate box for production type. If you check other, please identify type of production (Music video, documentary, short film, student film, etc.). Include the title of the production, publicist, director, producer, production manager and location manager. Location manager's cell phone must be included.

Item #8: TV Commercial name / product: If you are shooting a commercial or an industrial, please identify the product or client, otherwise leave this field blank.

Item #9: Insurance: Provide all detail in this section, even if your insurance is on file in our offices. Be sure to include the insurance broker and phone, policy number, general aggregate amount and expiration date. Proof of Insurance Coverage: Every production requesting a permit MUST provide an ORIGINAL certificate of insurance to be kept on file at the MOFTB, which contains the wording:

"City of New York is named as additional insured pursuant to, or with coverage at least as broad as, ISO Form CG 20 12 (07/98 ed.)"

If you have previously submitted an original certificate of insurance it is your responsibility to know the policy number and expiration date when requesting a new permit.

Signature and Proof of Identification:
Please sign and date, indicating your appropriate title. Applicant must provide photocopy of driver's license of passport. Permit must be signed by a full time employee or agent of the company. Student Films require a valid student photo ID, plus a letter from your film school naming you as a student, your project, and your filming dates. Only the student named in the letter may sign the permit. International productions require a valid passport and a letter from the production company identifying you as the representative of that company.

Your permit is valid ONLY with an original signature and an official seal from a location consultant at the MOFTB.

Print an original for signature and submit in person with a photocopy of your valid photo ID to our office:
