Everything you wish to do during your film shoot must be requested in writing on your Schedule "A". This information provides a clear picture of your activity - for the MOFTB, the police officer on the street, the residents living on the block where you are filming, the merchant whose business may be affected by your shoot, or even the Parks Department employee who has questions about your presence in the park.
Here are some guidelines for filling out a Schedule "A":
ALWAYS complete "NAME OF FEATURE", "TEL. NO." and "PERMIT NO." at the top of the form. If it's your first time applying for a permit, you will be assigned a permit number by a coordinator. You MUST type this number onto your shooting permit.
BOX 1: Under "LOCATION" type in the name and exact address (and cross streets) for interior scenes.
Example: BAR 85
85 Tenth Avenue
(btw W. 15th & 16th Streets)
Please be specific. Include the street and cross streets for exterior locations. Listing a general area such as Times Square, Greenwich Village, or Soho is not sufficient.
Example: Provost Street
(btw Kent & Java Streets)
Greenpoint, Bklyn.
Small productions that only have one or two vehicles and a small crew of 5 to 15 people that need to do quick establishing shots, host wraparounds or man-in-the-street interviews in a particular section of town may request a larger area. Please specify a grid of streets with north, south, east & west boundaries.
Example: West 14th Street to West 23rd Street
(btw 7th & 10th Avenues)
In the "DATE" column, fill in the date of your shoot for that location (including day of the week, month, date and year).
Example: Thursday,
Sept. 19, 2002
If you are filming at the same location for more than one day, each date does not need to be listed separately.
Example: Wednesday, Sept. 18 -
Friday, Sept. 20, 2002
In the "TIME" column, fill in the start and finish times of your proposed shoot (using the 12 hour clock and including "am" and "pm"). Please do not use military time.
In the "DESCRIBE SCENE IN DETAIL" box, indicate whether the action takes place in an interior or exterior locale. Name what the locale plays for in your script, and for exterior scenes, give an accurate, detailed description of the physical action in the scene.

Various interior dialogue scenes.
EXT. BROWNSTONE
Actor pulls up to curb in car. He exits auto
and is approached by a group of punks (4 extras)
One pulls a gun and asks him to hand over his wallet.
Mugger takes wallet and group flees on foot down sidewalk.
NOTE: Any use of firearms, weapons, police uniforms, police vehicles, holding of traffic, or conditions that may be deemed potentially unsafe, require police assistance which must be requested on the Schedule “A” form. Driving shots that require a tow rig, side mounts or multiple vehicles must have police assistance with a radio mobile patrol (RMP) car. Your production company is responsible for calling the Movie TV Unit (212) 489-6710, x219, to request an officer by 10:00am the day prior to your shoot. Shoots on the weekend (Saturday OR Sunday) needing police assistance must call on Thursday before 10:00am.
STUDENTS: Police assistance is not available for student films on the weekends or overnight.
BOX 2: This space should be used to request permission to hold parking for your vehicles at the location listed in BOX 1 if you need it. This is a privilege extended to productions with 10 vehicles or more, such as TV series, feature films or large commercials. (Requests to hold parking do not apply to productions with 1-2 vehicles)
In order to hold parking you need to list the street you wish to park on, the side of the street you wish to take (north, south, east or west) and the cross streets. Then you need to type in the parking regulations for that street exactly as they appear on the parking signs. Parking requests should be separated for picture and equipment vehicles.

REQUEST: Permission to clear & hold parking in advance for picture on:
Columbus Avenue, west side, btw W. 69th & 70th Streets. (N/S 7a- 10p, M-F)
REQUEST: Permission to clear & hold parking in advance for equipment trucks on:
West 69th Street, north side, btw Columbus Ave. & Bway.(N/P 8a- 11a,T&F)
Columbus Avenue, east side, btw W. 69th & 70th Streets. (N/P 7a- 10a,
1 hour meters 10a-7p)
BOX 3: Type in any other requests you may have for the location listed in BOX 1, such as staging equipment on the sidewalk and/or in the curblanes, police assistance, etc.
If your company is moving to a second location after the one listed in BOX 1, then type the following AFTER all your requests for the first location:

List the next location in BOX 4 and type in your parking requests and any other requests as you did for your first location.
If you're shooting at three or four locations in the same general area, and your trucks will remain parked in one place for the duration of the shoot, then list the three or four locations in the appropriate boxes (BOX 1 thru 3 or 4) THEN list your parking request and other requirements in BOX 5.
