Fingerprint Unit

Pursuant to the New York City Administrative Code §21-119 (derived from Local Law 60/1984), individuals who work at child care, home care, and family care facilities, including those entities that are under contract with the City of New York, must be screened for criminal convictions and/or pending criminal actions. All prospective hires must have their fingerprints checked against New York State and Federal criminal records databases. Notifications of arrests in New York State are provided on an ongoing basis for all active employees.

Please note, if you were previously fingerprinted by DOI after January 1, 1999, you do not need to be fingerprinted again.  Please see the section below regarding obtaining a duplicate letter.

Required documents:

  • Fully completed application, which must be obtained from your employer or program
  • Photo identification from the following list:
    • Driver's License or Non-Driver ID
    • United States Passport
    • Certificate of Naturalization or Alien Registration Card
    • Student Identification
    • Government Benefit Card
    • Other Government Issued Identification

Applicants may pay the $127.00 fingerprint processing fee by credit card, (MasterCard, American Express, VISA or Discover) or by Postal Money Order from the United States Post Office made payable to the New York City Department of Investigation (other types of money orders will not be accepted).  All applicants must submit an application with their original signature, as well as the Permit or Fiscal number of the organization for which they will be working.

Requests for duplicate letters:
Duplicate Letters are issued for individuals who were previously fingerprinted by the Department of Investigation after January 1, 1999, and have changed employers.

*Please note if you were fingerprinted prior to January 1, 1999, you will need to be fingerprinted again, and it will be necessary to schedule an appointment and pay a $127 fingerprint processing fee.  Options on how to schedule an appointment and make payment are outlined above.
