Confidentiality and Privacy Statement

This statement explains how the Registry treats personal information that is collected from enrollees and what it does to strictly protect the confidentiality of enrollees. "You" and "your" is used in this statement to refer to enrollees.

"Personal information" is information that identifies you, such as your name, address, email address, telephone number, date of birth and social security number (including the last 4 digits). "Contact information" is what we use to stay in touch with you, such as your home address, email address and telephone number.
