Ordering NYC birth and death certificates
To avoid waiting in line, place your order online. Please remember to bring valid photo identification.

| Payment: | $15 for each copy using a credit card, debit card, or check or money order (made payable to the NYC Department of Health and Mental Hygiene). Effective December 9 2011, an additional $2.75 fee will be charged per order for a security check ,(an order can include more than one certificate). |
Correcting a NYC birth and death certificate
Do not forget to bring valid photo identification. Also, remember that birth corrections for children 12 months of age or younger submitted through the hospital are free of charge and processed faster.
