Take Care New York: Ideas for Action
Take Care New York 2012 is a comprehensive health policy that can not be achieved solely by the Department of Health and Mental Hygiene. We cannot reach our goals unless our partners – health care providers, nonprofit organizations, faith-based organizations, other city agencies, businesses, policy-makers, and many others – take on these priorities with us. We offer these “Ideas for Action” so you can help us succeed.
“Ideas for Action” are innovative and achievable actions that can be implemented in your community or organizations.

Take Care New York Champions are partners who have demonstrated their commitment to Take Care New York by implementing activities, programs and/or policies that address the 10 agenda items.
Take Care New York Champion Partners will receive a:
- Take Care New York Champion certificate
- Feature on the DOHMH Take Care New York Web site
- Feature in the Take Care New York Newsletter
To become a Take Care New York Champion an organization must:
- Be a Take Care New York Partner. If you are not a partner, sign-up to become a partner by:
- Completing an On-line application or
- Calling 311 and asking for a Take Care New York application by mail.
- Implement 3 or more activities listed in the “Ideas for Action” in one calendar year.
- Submit* a brief summary of the “Ideas for Action” activities your organization has implemented by completing an Ideas for Action Summary Form (PDF).
* Submission of a summary is not necessary if you complete the Take Care New York partner survey.
