The New York City Community Health Survey (CHS) is a telephone survey conducted annually by The Health Department's Division of Epidemiology, Bureau of Epidemiology Services. CHS provides robust data on the health of New Yorkers, including both neighborhood and citywide estimates on a broad range of chronic diseases and behavioral risk factors.
CHS Summary Reports
These reports provide data tables and charts of CHS data on specific health topics and populations of adult New Yorkers by sociodemographic factors (such as age and borough).
