Tobacco Quit Kit
Everyone Loves a Quitter. Smoking doubles the risk of death in every age group, killing more than 7,500 New Yorkers every year. More than two thirds of smokers want to quit, and more than half try to stop every year; however, fewer than 10% are successful over the long term without assistance. Quitting smoking is the single most important thing a person can do for their health.
Being Tobacco-Free is one of the ten key priorities of New York City's Health Initiative: Take Care New York. This Tobacco Quit Kit was created to provide you with valuable tools and information to help your patients become, and remain, tobacco-free. We hope you use these materials and talk with your patients about quitting smoking. Materials in your Quit Kit include:

- Smoking Questionnaire (PDF): This patient self-screener can assist you in
assessing your patients' smoking habits and readiness to quit.
- Vital Signs Chart Stickers (PDF): Use these stickers in the medical record to
incorporate tobacco status into your patient's vital signs. Ask your
receptionist, medical assistant or nurse to place one at the top of the visit
notes.
- Adult Preventive Care Flow Sheet (PDF): To help monitor and document physical
examinations, test results, counseling, and immunizations.
