Birth and Death Certificates

Quick Tips

Order a birth or death certificate online using your personal credit/debit/checking account if your name appears on the records so you don't have to wait in line.  UPS express mail delivery is available.

If you're ordering a birth certificate by mail, you must include identification and have the application notarized.

If you're ordering a birth certificate for a deceased relative, you must do by mail or in person.

If your child is under 12 months of age, the fastest way to correct a birth certificate is to bring the original certificate and a completed correction application (PDF) to the hospital where your child was born.

Check the processing time for your order.
