The most convenient way to get a birth or death certificate is to order it online. UPS express mail delivery is available.
Order In Person
Where: 125 Worth Street (Room 133) in Lower Manhattan between between Lafayette and Centre Streets. Enter on Centre Street or Lafayette Street (for handicapped access)
When: Monday - Friday except holidays, 9 a.m. to 3:30 p.m. Lines are shortest in the morning and midweek.
Your wait time will be reduced if you use a credit or debit card at one of the kiosks in the lobby. Long-form birth certificates and death certificates will be mailed to you.
Whether or not you can walk out with your birth certificate depends on the type of identification you bring:
- If you bring Category 1 identification, you can leave with the birth certificate in most cases.
- If you bring Category 2 identification, the certificate will be mailed to the address provided on your documents.

Order By Mail
To get a birth or death certificate by mail, please:
- Complete and sign the birth or death certificate application. If you are unable to download the application, please call 311 or 212-639-9675 outside New York City to request one.
- Have your application notarized.
- Enclose the identification indicated on the form.
- Calculate the fees and enclose your payment (see below).
- Include a self-addressed, stamped envelope.
- Mail your application package to the address below.
- Check the processing time for your order.

Correct By Mail
To correct a birth or death certificate by mail, please:
- Complete and sign the birth or death certificate correction application. If you are unable to download the application, please call 311 or 212-639-9675 outside New York City to request one.
- Enclose copies of identification indicated on the form.
- Enclose original documents to support the correction + 1 copy of each of these documents.
- Calculate the fees and enclose your payment (see below)
- Include a self-addressed, stamped envelope.
