Birth & Death Certificates Fees

  • Birth and death certificates cost $15 per copy.  Certificates of miscarriages and still births also cost $15.

    Processing fees vary depending on how you order the certificate and may include a search for 3 consecutive years.  It costs $3 for each additional year searched.

    • Online:          $8.30
    • In-person:     $2.75 per order (see table below)
    • Mail order:     No charge, unless extra search is required

    If a certificate isn't found, the Bureau of Vital Records (BVR) will issue a "Record Not Found" statement and the customer will be charged $15 and refunded the cost of any additional certificates ordered.  If the customer returns the "Record Not Found" statement by mail or in person with the additional information requested within 3 months from the issue date, BVR will conduct an additional search at no cost.

    If a death certificate is issued with the cause of death listed as "pending further study," BVR will waive the $15 fee for an updated certificate if you submit the original death certificate with a new application within 3 months of the medical examiner submitting the final cause of death. The Office of the Chief Medical Examiner determines the cause of death.

    Refund Policy

    Credit Card Orders

    If the Bureau of Vital Records (BVR) reviews and cancels a certificate order within 1 business day (usually for reasons related to eligibility), the customer's account is credited for the cost of the order and the shipping charge but not the $8.30 processing fee.  The customer receives an e-mail from BVR explaining the cancellation and told how to contact BVR with further inquiries.  Once an order has been cancelled, the customer must re-submit it according to the instructions provided.

    If the order is accepted and searched, this process can take from 5-7 business days.  If the certificate cannot be found, the customer is mailed a "Record Not Found" statement.  A refund check, minus the $15 cost of the "Record Not Found" statement and the processing and shipping fees is issued separately.  Check refunds take approximately 30 days.

    If the customer submits the "Record Not Found" statement with the additional information requested within 3 months of the cancelled order, BVR will do another search at no additional cost.

    Please note: 

    "Record Not Found" statements cannot be used for online orders; they must be returned by mail or in person.

    Other Orders

    Refunds do not usually occur for in-person or mail orders because the Bureau of Vital Records does not cash a person's check or money order unlesss the order can be filled.  However, if a customer overpays, a refund is made for overpayments of $3 or more.  All documents are returned with cancelled orders.

    Birth Certificate Corrections

    The Health Department charges a non-refundable $40 processing fee to make most birth certificate corrections (see below).  Copies of corrected certificates cost $15 each.  This $15 is waived if you enclose a certified copy of a certificate that has been issued within the past 3 months and you ask to exchange it for 1 corrected certificate.

    $40 Processing Fee Applies:

    • Adding a child's given name by family more than 60 days after birth
    • Family's errors and omissions, except when adding a given name within 60 days of birth (omissions include adding a parent who was married prior to the birth of the child)
    • Hospital and licensed midwife errors and omissions after 12 months
    • Adoptions (court order)
    • Correction of sex after 1 year of age
    • Re-submitting an application more than 1 year after rejection

    No Fee Applies:

    • Administrative error by the Health Department
    • Acknowledgments of paternity
    • Orders of filiation (paternity)
    • Establishment of parentage
    • Adding a child's given name if submitted by the:
      • family within 60 days of birth to the Health Department
      • hospital of birth within 12 months of birth
    • Delayed registration of birth (call 311 to request form VR34)
    • Report of foundling
    • Miscarriage or still birth certificate changes

    Death Certificate Corrections

    • Submitted by funeral director
    • Submitted by hospital more than 12 months after death
    • Resubmitted application more than 1 year after rejection

    No Fee Applies:

    • Administrative error by the Health Department
    • Submitted by the hospital within 12 months of death
    • Adding missing information
    • Miscarriage or still birth certificate changes
