Influenza Pneumococcal Information

Perinatal Hepatitis B Prevention Program
  • The program identifies hepatitis B positive pregnant women, conducts case management of infants at risk for perinatal hepatitis B infection and works with hospitals to achieve universal birth dose coverage and documentation of the birth dose in the Citywide Immunization Registry.
  • To report Hepatitis B in Pregnancy to the Bureau of Immunization, go to Hepatitis B/Perinatal Hepatitis B webpage.

Citywide Immunization Registry (CIR) Newsletter

Vaccine Information Statements
Under the National Childhood Vaccine Injury Act, all health care providers are required to use Vaccine Information Statements (VIS) to inform parents, guardians, legal representatives, and adult patients about the benefits and risks of vaccines prior to vaccination.

Lifetime Health Records
The Lifetime Health Record can only be ordered by providers giving healthcare to children. To order the Lifetime Health Record, call the Health Department Call Center at (917) 438-9766. Parents who need to replace a lost Record should contact their child's provider.
