CIR: Online Registry Access

To access the Online Registry, each health care facility or practice designates a Security Administrator (User Manager). The Security Administrator (User Manager) must read, sign, then mail or fax

(a) the Security Administrator (User Manager) User ID/Password Request Form, and (b) the Acceptable Use Protocol and Security Administrator (User Manager) Confidentiality Statement for Online Access.

If your site is a facility (such as hospitals and clinics) please include letterhead for your facility with the Online Registry Request form and Security Administrator (User Manager) Confidentiality Statement.

Next, the Online Registry assigns a User ID and Password to the Security Administrator (User Manager). The Security Administrator (User Manager) can then set up User IDs and Passwords for additional facility or practice staff members. He or she must have each additional user read and sign a User Confidentiality Statement for Online Access and keep those on file at the site.

The Department of Health and Mental Hygiene offers free training for all health care providers on how to gain online access to the Registry. To arrange for training at your office, call (347) 396-2400.

For Private Practices:

For Facilities (such as hospitals and clinics):
To access the Online Registry, the director /owner/licensed pharmacist in charge of the facility designates a Security Administrator (User Manager). The Security Administrator (User Manager) must complete the Security Site Administrator form, and the Confidentiality Statement, then mail or fax them back to the CIR.

For independent pharmacies administering immunizations, but that are not affiliated with a medical facility:

To access the Online Registry, the director /owner/licensed pharmacist in charge of the facility designates a Security Administrator (User Manager). The Security Administrator (User Manager) must complete the Security Site Administrator form, and the Confidentiality Statement (PDF), then mail or fax them back to the CIR. For Agencies requesting Read-Only Access (Foster Care Agencies, Managed Care Organizations, WIC):
  • To access the Online Registry, the director of the agency designates a Security Administrator (User Manager). The Security Administrator (User Manager) must complete the, Security Site Administrator form, page 2- and the Confidentiality Statement, page 3 from the Read-Only Access Forms and Quickguide (PDF) then mail or fax them back to the CIR.

For Providers Outside of NYC requesting Read-Only Access:
  • To access the Online Registry, the director or clinical provider in charge of the agency designates a Security Administrator (User Manager). The Security Administrator (User Manager) must complete the, Security Site Administrator form, page 2- and the Confidentiality Statement, page 3 from the Read-Only Access Forms and Quickguide (PDF) then mail or fax them back to the CIR.

Schools, Child Care Centers, Camps may sign up for Online Registry
