Submitting The Application for Permit

If the applicant cannot represent him or herself throughout the application process, then he or she must provide Power of Attorney to a representative, in the form of a standard legal form (e.g., Blumberg-Statutory Short Form for Power of Attorney (M-51)) or the enclosed Affidavit Granting Authority to Act. In addition to submitting documents on behalf of the applicant, the representative may also be authorized to sign the application form. The person representing the applicant must also provide one form of his or her own photo ID, as detailed in the Acceptable Photo Identification section on the Application Checklist.

Submit completed Application for Permit and supporting documents (see Application Checklist) in person to the Citywide Licensing Center. The application package will be reviewed to ensure that all forms are correctly filled out, and that all required documentation is presented.

If all required documentation is not provided as part of the application package, you will be instructed as to what additional information is needed. If an application package is not complete, it will not be accepted, and you will not be allowed to pay the permit fee.

Upon determination that an application package is complete, the reviewer will instruct you to pay the appropriate fee. The permit fee is payable by money order or check to the New York City Department of Health.

Once the fee has been paid, an inspection will be scheduled within one to three weeks.

Religious, fraternal and charitable organizations which provide food services more often than once a week must obtain a permit.

Any such organization providing food service less frequently than once a week must notify the Department in writing of its intention to engage in such food service operations and must obtain authorization from the Department (Certificate of Registration). Notification to the Department or Citywide Licensing Center, must be in the form of a complete Application for Permit form and supporting documentation. Such authorization may be issued for a term not to exceed two years and the payment of a fee is not required.
