Retail Frozen Dessert Manufacturing (MFD)
The authority to manufacture and sell frozen desserts at retail must be reflected in the appropriate Food Service Establishment permit or in conjunction with a Temporary Food Service Establishment processing permit.
Request to add said authority to a pre-existing Food Service Establishment permit must be made by submitting to the Citywide Licensing Center a completed "Application for Permit" form along with the existing permit.
Write "MFD addition to permit #H__-_______" on the Application for
Permit form next to "Name of Permit".
