The mission of the Bureau of Immunization is to prevent the occurrence and transmission of diseases through immunization. The Bureau promotes the immunization of children and adults against Hepatitis B, Measles, Mumps and Rubella, Varicella, Diphtheria, Tetanus, Pertussis, Haemophilus Influenzae Type B, Polio, Influenza and Pneumococcal disease.

  • For provider or consumer questions and for information on the location and hours of the Walk-In Immunization Clinics call 311.
  • For information about where to obtain a flu shot, call 311.

Surveillance

  • For providers to report a vaccine-preventable disease: (212) 676-2284 or (212) 676-2288
  • Hepatitis B in a pregnant woman or newborn should be reported directly to the Perinatal Hepatitis B Prevention Program: (718) 520-8245

  • Citywide Immunization Registry
  • Serves as a database providing immunization information on children and adolescents 18 years of age and younger. Call to enroll in the CIR, access immunization records, or for more questions: 212-676-2323
