In March 2004 the Department launched Take Care New York, which addresses the leading preventable causes of illness and death in ten priority areas. The Take Care New York approach expands existing programs, promotes evidence-based interventions, coordinates services, builds partnerships, and advocates for social and economic changes that produce health benefits. Goals include increasing the number of New Yorkers who have a doctor; reducing tobacco use; decreasing the number of HIV/AIDS-related deaths; reducing the number of drug-related deaths; increasing the proportion of New Yorkers screened for colon cancer, breast cancer, and cervical cancer; raising the percent of seniors who are immunized; decreasing the incidence of childhood lead poisoning; and reducing infant mortality rates.
Community Collaborations
Transitional Health Care Coordination (THCC) works at Rikers Island and in the community to provide health care education, community referrals, and follow-up for detainees and inmates released from City correctional facilities and also for their families.
