Public Health Detailing Program : NYC DOHMH

Public Health Detailing Program

The Public Health Detailing Program works with primary health care providers to improve patient care around key public health challenges. Department of Health and Mental Hygiene (DOHMH) representatives promote clinical preventive services and chronic disease management through the delivery of brief, targeted messages to doctors, physician assistants, nurse practitioners, nurses, and administrators at their practice sites. “Detailing Action Kits” – containing clinical tools, resources for providers and patient education materials to promote evidence-based best practices – are distributed during visits. Modeled after the pharmaceutical sales approach, the Public Health Detailing Program builds on the DOHMH's extensive experience in medical provider education, health care quality improvement and community-based health promotion.

The Public Health Detailing Program is organized around specific clinical topics, chosen largely because of their anticipated impact on morbidity and mortality. To date, detailing campaigns have focused on the following health issues: influenza vaccination, colon cancer screening, smoking cessation, asthma, diabetes, contraception, hypertension, depression screening, cholesterol, alcohol screening and brief intervention, HIV testing, Electronic Health Records (EHRs),  Identifying & Reporting Child Abuse & Neglect and Nurse-Family Partnership, Obesity,Medical Reserve Corps, and Tobacco RetailingSmoking Cessation to Mental Health ProvidersIntimate Partner ViolenceColon CancerBreastfeedingObesity in Children, Influenza and Pneumococcal Vaccination 2009-2010. Topics of future campaigns include Medical Adherence.

The Public Health Detailing Program targets three communities burdened by poor health: East and Central Harlem, North and Central Brooklyn, and the South Bronx. District Public Health Offices (DPHOs) established in each of these neighborhoods serve as the home of the Public Health Detailing Program. Occasionally, detailing campaigns are conducted citywide, as was the case with the tobacco cessation campaign, diabetes campaign, and the HIV testing campaign.

This program is funded in part with private support from The Mayor's Fund to Advance New York City.  The Mayor's Fund is a not-for-profit organization established to strengthen public programs with support from individuals, corporations, and foundations.

Influenza and Pneumococcal Vaccination 2009-2010 Action Kit
Every year influenza-related illness, including pneumonia, causes 36,000 deaths nationwide. Here in New York City, influenza causes an estimated 1,100 deaths per year, and respiratory infections as a whole cause more than 2,000. Health care practices should ensure that at-risk patients – and all staff – receive both the seasonal vaccine and the novel H1N1 vaccine this fall. Obesity in Children Action Kit
In New York City (NYC), 43% of elementary school children are at an unhealthy weight; more than half of these children are obese. This is a much larger proportion than expected based on the national rate. In our public high schools, 28% of the students are either overweight or obese . Overweight children have many risks, including hyperlipidemia, hypertension, type 2 diabetes, social stigmatization, and psychosocial difficulties. Providers should monitor weight status and consistently offer counseling on healthy eating and exercise to all children and adolescents, even those who are at a healthy weight.

Breastfeeding Action Kit
Exclusive breastfeeding for the first 6 months of an infant's life has many short- and long-term health benefits for babies and mothers, including a reduction in infectious diseases and mortality during infancy, improved bonding, and maternal postpartum weight loss. Despite these benefits, relatively few women breastfeed for the recommended time period. In NewYork City (NYC), nearly 85% of postpartum women initiate breastfeeding, but at 2 months only 32% are still exclusively feeding their infant breast milk. This is far below the Healthy People 2010 goal for 60% of women to breastfeed their babies exclusively for the first 3 months.9

In New York City almost 1500 people die each year from colon cancer. The best way to test for – and prevent – colon cancer is colonoscopy. Many more people are getting colonoscopies; however 4 in 10 eligible patients are still not optimally screened for this largely preventable cancer.

Intimate Partner Violence (IPV)
Intimate Partner Violence (IPV) refers to a pattern of physical, psychological, sexual and/or economic abuse by a current or former partner. Also known as domestic violence, IPV is a serious public health problem. Every year, approximately 4,000 women and 900 men are treated in NewYork City emergency rooms for IPV. Almost half (44%) of women killed in NYC each year are killed by their intimate partners. Pregnancy may be a time of increased risk for IPV. In a survey of 2,000 NYC women who gave birth in the past year, approximately 4% reported experiencing physical IPV during pregnancy. Aside from the risk of injury and death, IPV significantly increases the risk of other mental and physical health problems, including cardiovascular disease, diabetes, depression, stroke and asthma. If undetected, IPV can result in misdiagnoses and inappropriate treatment or management of these health conditions.

Smoking Cessation to Mental Health Providers
Smoking is the leading preventable cause of death in the United States (US). Up to one half of life-long smokers, depending on age, are expected to die of tobacco-related diseases. Smokers who die of tobacco-related diseases lose an average of 14 years of life. In New York City (NYC), smoking kills about 8,000 people a year, a third of them before age 65. Quitting at any age reduces the risk of tobacco-related diseases and prolongs life.

Tobacco Retailer Kit

Tobacco Retailing Kit
Under the New York State (NYS) Adolescent Tobacco Use Prevention Act (ATUPA) and the New York City (NYC) Tobacco Product Regulation Act, retailers who sell tobacco to persons under 18 years old can lose their New York State license to sell tobacco, have their New York State lottery license suspended for 6 months and/or be fined $4,000. This kit will help tobacco retailers and their employees follow these laws.

The New York City Medical Reserve Corps (NYC MRC) is a key component in that effort. The NYC MRC bolsters the city’s emergency preparedness by training a group of volunteer health professionals who are ready to respond to health emergencies.

About two thirds of the US population are overweight or obese, including more than half (3,000,000) of the adults in NYC. Fortunately, obesity is preventable and as obesity rates decrease, so will chronic disease and associated mortality.
