How to Use the Program Handbook

The purpose of the Program Handbook is to provide supplemental materials for the participants of the Creating a Medical Home for Asthma program. It is not intended to be a self-instructional document. Rather, it is offers information that allows the participant to be better prepared for each of the program sessions and to refer to key concepts and handouts discussed at these sessions.

The materials for each sessions is organized into four sections:

Bullet Handouts

Bullet Assignment

Following each session, the participant is encouraged to review that session's key concepts and the handouts. It is also important for the participant to focus on the assignment, as well as to review the agenda and key concepts for the next session.
