Key Concepts
Understanding the Team Approach to Asthma Management
Using the information learned from Session
1, clinic staff should now begin thinking about how to turn the
clinic into a "medical home for asthma." Having an
understanding of the team-based approach for the CMHA program
is critical to this concept.
All staff members understand and can talk about asthma
Parent of a child with asthma works with staff
Staff encourage parents to take an active role in learning to control asthma
Family's
observations are necessary
To evaluate if the treatment plan is workable
To evaluate how
the treatment plan is working
To make medication
adjustments
Take time
To make the family feel comfortable
To listen to the
family's concerns
To make sure family
understands:
How to give medication correctly
The name of the medication and how it is supposed to work
Prevention or control of symptoms
Why it should be continued after the immediate attack is over
What should be done in an emergency
That you want to be called:
- If the treatment plan is or isn't working
- If they had to go to another provider
- If they have any questions or concerns

Handout 2-1 (Activity II)
Asthma Care in "Creating a Medical Home
for Asthma" Clinics |
Summary
1. Clerical staff screens patients, is alert
to asthma symptoms, and feels comfortable adding information
on the screening form.
2. MD and RN ask Mrs. Jones questions to gather
information and bring out her concerns about George's asthma.
3. MD and RN teach Mrs. Jones and George several
basic messages:
Asthma attacks can be prevented with the right treatment
George
should be able to play hard without symptoms
Mrs. Jones can help control George's asthma by observing what
triggers it and avoiding or removing triggers
Asthma
is variable and periodic adjustments in treatment are needed
Non-urgent, scheduled appointments can help the medical team
and the family plan to control George's asthma
4. Phone calls are forwarded to MD for instructions
and adjustments in medications.
5. There can be multiple triggers for asthma,
that is: viral infections, ear infections, cold, or sometimes
we don't know what the trigger is.

Handout 2-2 (Activity V)
Worksheet
Issues to be Analyzed
List specific issues to be analyzed: how do
we turn our clinic into a Medical Home for Asthma?
| Factors helping resolution of the issue
|
Factors hindering resolution of the issue

Handout 2-3 (Activity V)
Individual Assignment Sheet
In order to bring about planned change, you
can either increase the helping or diminish the hindering
forces.
1. Choose 1 helping force that your
clinic team can realistically do something about, and
write it here. (Choose 1 that you think will have the most impact
in helping you reach your goal.)
2. Choose 1 hindering force that your
clinic team can realistically do something about, and
write it here. (Choose 1 that you think has the most impact in
keeping your team from reaching the goal.)

Handout 2-4 (Activity V)
In order to bring about planned change, you
can either increase the helping or diminish the hindering
forces.
1. As a group, choose 1 helping or hindering
force your group will work on. Write it down.
2. What specific
things can be done to reinforce or bolster the helping force
or diminish the hindering force?
3. Who
in your clinic will be responsible for implementing these steps?
4. What are the available
resources for implementing this step?
5. What is a reasonable time frame?
