Key Concepts

Understanding the Team Approach to Asthma Management

Using the information learned from Session 1, clinic staff should now begin thinking about how to turn the clinic into a "medical home for asthma." Having an understanding of the team-based approach for the CMHA program is critical to this concept.

Handout 2-1 (Activity II)

Summary

1. Clerical staff screens patients, is alert to asthma symptoms, and feels comfortable adding information on the screening form.

2. MD and RN ask Mrs. Jones questions to gather information and bring out her concerns about George's asthma.

3. MD and RN teach Mrs. Jones and George several basic messages:

Bullet Asthma attacks can be prevented with the right treatment

BulletGeorge should be able to play hard without symptoms

Bullet Mrs. Jones can help control George's asthma by observing what triggers it and avoiding or removing triggers

BulletAsthma is variable and periodic adjustments in treatment are needed

Bullet Non-urgent, scheduled appointments can help the medical team and the family plan to control George's asthma

4. Phone calls are forwarded to MD for instructions and adjustments in medications.

5. There can be multiple triggers for asthma, that is: viral infections, ear infections, cold, or sometimes we don't know what the trigger is.
