Introductions
 Purpose: 1. To introduce the speaker and participants
2. To provide an overview of the activities  
Time needed:  10 minutes
Training team:  Entire team 
Materials needed:

1. Name tags
2. Tent cards (placards placed in front of participants, if seated at a table)

Each person will need:
1. A pen or pencil
2. A sheet of paper

Seating arrangements: No special arrangement required.
Options:
1. Arrange chairs in a circle to encourage participation and inclusion of everyone present

Background Information
 Purpose: 1. To provide a rationale for the Creating a Medical Home for Asthma Intervention
2. To provide statistics of prevalence of asthma in US  
Time needed:  20 minutes
Training team:  Educator/facilitator
Materials needed:

Trainer:

1. Overhead or LCD projector
2. Overheads or PowerPoint slides with statistical information (national and local data)

 Purpose:
  1. To provide an overview of asthma and treatment plans
  2. To identify goals/objectives of treatment
  3. To understand how to assess family's fears
  4. To provide a description of the team-based approach
  5. To provide an example of a successful case in the clinic
Time needed:  20 minutes
Training team:  Clinician(s)
Materials needed:

Trainer:

  1. Overhead or LCD projector
  2. Slides or overheads ("Overview of Asthma Treatment")

Activity VII - Break


 Activity VIII
Identification of Problems and Issues
 Purpose:
  • To get everyone involved in understanding that they are a vital part of the program and should do all they can to make it successful
  • To identify specific concerns of the participants so they can be addressed in future sessions
Time needed:  60 minutes
Training team:  Health educator/facilitator
Materials needed:

Trainer:

  1. Masking tape
  2. A pad of newsprint (large paper to write down the findings for the entire group to see)
  3. As many felt tip markers as you have questions to be answered

Each person will need:

  1. A pen or pencil
  2. A sheet of paper prepared ahead of time with one of the questions to be asked (One sheet is needed for each person taking part)
Seating arrangements: Set up chairs in groups of 2 rows facing one another with the same number of chairs in each row as there are questions to be asked

Closing Remarks
 Purpose:
  • To provide an overview of the activities in the current session
  • To provide closure to current session
  • To prepare participants for the activities for the upcoming session and remind them of any assignments
Time needed:  5 minutes
