The Evaluation Plan is designed for the CMHA
training facilitator (or program coordinator), as well as for
clinic management, to use as a guide for the assessment of the
CMHA Program. It serves as a framework for understanding the
steps necessary for conducting the program evaluation according
to the clinic's evaluation objectives.
Presented in five parts, this document begins
with a brief introduction about why program evaluation is helpful
for determining if the CMHA Program is doing what it should be
doing. The second part consists of a discussion on evaluation
design considerations that are based on the PRECEDE-PROCEED Model.
The next section lists key measurement domains that can be used
when conducting evaluations at three group levels-patients, staff,
and organization. This is followed by an outline of activities
to be done by clinic management for implementing the Plan. Lastly,
the Evaluation Plan provides examples of evaluation forms that
could be used at the patient, staff, and organizational levels.
