Welcome to the New York Citywide Immunization Registry (CIR)! The CIR keeps immunization records for New York City's children.

Our Mission: To create a complete and accurate database of immunization records that is accessible to health care providers, parents, and authorized agencies to help ensure that New York City children receive all required immunizations and are, thereby, protected from vaccine-preventable diseases.
