Enroll Your Child in the CIR
Instructions for forms: Choosing a form will open a new window for downloading or printing. To return to this page, simply close the new window. Mail or Fax completed forms to the CIR. (For instructions on opening, saving, and printing the PDF files below, click here.)
Correct or Update Your Child's Record
Please follow the instructions below:

2. Mail or Fax the completed Child Enrollment/Update Form and Lifetime Health Record to the CIR.
Request Your Child's Record.
Parents or guardians may request immunization records for their child from the Citywide Immunization Registry (CIR) by completing the Parent Request for Immunization Record application and mailing the form to the CIR. Record requests take approximately two weeks to process.

If no record is found or a record without immunizations is found in the CIR, parents/guardians are told immediately and encouraged to contact their child's health care provider to request that the immunization history and future immunizations be reported to the CIR. In addition, parent/guardians will be offered the opportunity to enroll their child.

Enroll Your Child in the CIR
Parents or guardians may enroll or update their child in the Citywide Immunization Registry by completing and mailing the Child Enrollment/Update form and attaching a clear copy of their child's Lifetime Health Record or other immunization card.

Please note that the information in the Registry is strictly confidential. Access to immunization records in the CIR is limited to licensed health care providers, parents or legal guardians/custodians, health officials and authorized agencies only. The CIR does not release any identifying information beyond that which is already known to the provider, parent, legal guardian/custodian or authorized agency.
