Register
Providers of childhood immunizations need to be registered in order to report immunizations and gain access to children's records in the CIR.
- If you are a Private Practitioner, you can register online or by calling (212) 676-2323.
- If you are a Facility or Clinic, please call (212) 676-2323 to register.
Update Contact Information:
By notifying us of changes in your contact information (such as staff changes, new address or telephone number) you will be able to receive updates regarding the CIR and general immunization information.
