DOHMH Colonoscopy Direct Referral Initiative
About Direct Referral for Screening Colonoscopy
The New York City Department of Health and Mental Hygiene and the Citywide Colon Cancer Control Coalition (C5) encourage the use of direct referrals for screening colonoscopy for appropriate patients.
Direct referral for screening colonoscopy streamlines the referral system and improves patient adherence to screening recommendations by eliminating the pre-procedure consultation for medically eligible patients. Primary care physicians, who are most familiar with each patient’s medical history, can use the DOHMH/C5 “Direct Referral for Screening Colonoscopy” form (PDF) to assess their patients and identify those who are eligible for the process. A referral that takes only one visit instead of two is more likely to result in a patient following through with the colonoscopy.
