The Medication Adherence Project (MAP)
Toolkit and Training Guide for
Primary Care Providers and Pharmacists
If you are a pharmacist, health care provider, or clinic administrator
interested to help your patients improve medication adherence, read on!
From 2007 to 2009, the Medication Adherence Project (MAP) was developed and
implemented by the Cardiovascular Prevention and Control Program and the Fund
for Public Health in New York (FPHNY), with funding from the Physicians
Foundation. The project provided training, resources and longitudinal support to
clinicians and pharmacists working in busy primary care practices.
Ninety participants from 28 health care sites and pharmacies around New York
City participated in MAP trainings. Post-training surveys showed that providers
were more satisfied with patient interactions after participating in MAP.
Although MAP no longer provides direct trainings, a MAP toolkit and training
guide was developed to support practices wishing to better engage patients in
efficient and productive conversations about medicine-taking.
The Objectives of the Training are:
- To familiarize health care professionals with common barriers to
medication adherence faced by patients with chronic disease;
- To provide evidence-based solutions to improving adherence;
- To train health care professionals to engage patients in conversations
about medicine-taking;
- To assist health care professionals to integrate MAP tools into their
sites; using strategies and tools based on Quality Improvement methodologies;
and
- To assist health care professionals to integrate MAP tools into their
sites; using strategies and tools based on Quality Improvement methodologies;
and
The MAP Toolkit and Training Guide Contains:
- A two-part guide, designed to help the health care team put the MAP tools
into practice:
- Part One of the guide contains medication adherence basics, instructions
for using each clinical tool, instructions for incorporating the tools in
your workflow, and suggestions for introducing tools to your
colleagues.
- Part Two provides a train-the-trainer curriculum that guides you in
implementing MAP training sessions for your onsite colleagues.
