Colonoscopy Screening Database
The Colonoscopy Screening Database is a user friendly tool that Patient Navigator programs use to collect data for research and program evaluation. The data collected includes patient demographics, navigator activities, and colonoscopy/biopsy results. The report function gives users access to the following summary reports:

  • Colonoscopy Activity Report
  • Colonoscopy Tracking Report
  • Call list for managing patient reminders
  • Excel Spreadsheet reporting data on 60 patient variables

Getting Ready for Screening (PDF)
This form provides a patient referred for a colonoscopy with information about the navigator program. This is a helpful tool for physician’s office referring patients to a procedure site with a navigator program. The navigator’s contact information is listed on the form in case the patient needs additional information about the colonoscopy.

Direct Endoscopy Referral (DERS) Form (PDF)
This form is used by PCPs to refer average risk individuals directly for screening colonoscopy, avoiding an unnecessary visit for a GI consultation before screening.
