Getting
Ready for Screening (PDF)
This form provides a patient referred
for a colonoscopy with information about the navigator program. This is a
helpful tool for physician’s office referring patients to a procedure site with
a navigator program. The navigator’s contact information is listed on the form
in case the patient needs additional information about the colonoscopy.
Direct
Endoscopy Referral (DERS) Form (PDF)
This form is used by PCPs
to refer average risk individuals directly for screening colonoscopy, avoiding
an unnecessary visit for a GI consultation before screening.
