Colonoscopy Screening Database
The Colonoscopy Screening Database is a user friendly tool that Patient Navigator programs use to collect data for research and program evaluation. The data collected includes patient demographics, navigator activities, and colonoscopy/biopsy results. The report function gives users access to the following summary reports:

  • Colonoscopy Activity Report
  • Colonoscopy Tracking Report
  • Call list for managing patient reminders
  • Excel Spreadsheet reporting data on 60 patient variables

Colonoscopy Patient Navigation Resource Kit (PDF)
This kit is designed to help hospitals launch a colonoscopy patient navigator program. It contains detailed information on how to establish and run a navigator program, as well as identify and train patient navigators. It also includes samples and prototypes of documents hospitals can customize to meet their needs.

This is a flow chart for the Navigation Process in a hospital setting.

My Hospital Flow Chart (WORD)
This tool helps navigators create their own hospital flow chart.

Review of protocols and action steps (WORD)
This tool helps navigators evaluate the effectiveness of their interventions during the navigation process.

In reach assessment tool (WORD)
To increase colonoscopy volume, navigator programs should build relationships with all ambulatory clinics with patient populations likely to require screening, as well as hospital system community-based medical clinics. In addition to primary care, this can include ob-gyn, urology, gerontology and HIV. This tool is designed to help navigators and supervisors plan in-reach activities.

Appointment tracking form (WORD)
This tool helps navigators plan and track follow-up calls to patients.

Intake Form (WORD)
This form helps Navigators collect data on patient demographic information, medical history, referral, and follow-up activities.

Getting Ready for Screening (WORD)
This form provides a patient referred for a colonoscopy with information about the navigator program. This is a helpful tool for physician’s office referring patients to a procedure site with a navigator program. The navigator’s contact information is listed on the form in case the patient needs additional information about the colonoscopy.
