friendly tool that Patient Navigator programs use to collect data for research
and program evaluation. The data collected includes patient demographics,
navigator activities, and colonoscopy/biopsy results. The report function gives

Patient Navigation Resource Kit (PDF)
This kit is designed to
help hospitals launch a colonoscopy patient navigator program. It contains
detailed information on how to establish and run a navigator program, as well as
identify and train patient navigators. It also includes samples and prototypes
of documents hospitals can customize to meet their needs.

To increase colonoscopy volume,
navigator programs should build relationships with all ambulatory clinics with
patient populations likely to require screening, as well as hospital system
community-based medical clinics. In addition to primary care, this can include
ob-gyn, urology, gerontology and HIV. This tool is designed to help
navigators and supervisors plan in-reach activities.

helpful tool for physician’s office referring patients to a procedure site with
a navigator program. The navigator’s contact information is listed on the form
in case the patient needs additional information about the colonoscopy.
