Director’s Information and Forms for applying for a summer camp permit
The Department of Health and Mental Hygiene is dedicated to protecting the health and safety of all children who attend camps in New York City. The Department is making available online the New York State and New York City Health Codes for Children's Camps, as well as other required information, to ensure that your camp operates safely this summer.
All camps that received a permit the previous season are mailed a Renewal Camp Application Booklet to the mailing address used that season. If your camp has had a change of address or you are a new camp applying for a permit, you must use the New Camp Application Booklet.
Once the Summer Camp Program has reviewed and approved your permit application, you will receive your permit from the Citywide Licensing Center via mail. Camps should not operate until the permit application requirements are met and the permit is approved.
